Bednet Utilization Survey in the Philippines Bednet Utilization and Knowledge-Attitude-Practice Survey among Selected Barangays in 40 41 Malaria Endemic Provinces in the Philippines Survey Protocol Introduction: The Philippines has 80 provinces comprising 1,634 municipalities. These municipalities are further subdivided into barangays (villages) which is the smallest administrative unit. At the start of 2003, malaria was endemic in 66 provinces in the Philippines. The population at risk, defined at that time as the entire population of the province where malaria cases were found, was estimated to be 70,687,400. By 2010, malaria was endemic in 57 provinces and 843 municipalities in the Philippines. The population at most risk was 6,387,734, or 12.5% of the total national population. At this time, the population at risk was defined as the people living in barangays with stable, unstable and sporadic transmission based on the most recent stratification system of the National Program. By 2013, there was further reduction in the number of malaria endemic provinces to 53. Just 47 higher-incidence municipalities in 13 provinces contributed about 97% of the total malaria cases reported in 2011-2013 Malaria has historically been one of the 10 leading causes of morbidity and mortality in the Philippines. Over an 8-year period, and in particular, the recent years of 2010-2013 – there has been a significant reduction in malaria cases, annual parasite incidence (API) and mortality rates throughout most of the country (Malaria Program Review 2013). Incidence of malaria has declined rapidly due to the intensified strategies implemented in the endemic areas by the Philippine Department of Health (DOH) and Local Government Units with the support from The Global Fund (TGF), World Health Organization (WHO), Pilipinas Shell Foundation Inc. (PSFI) and other partners. Recent report indicates malaria cases to be less than seven thousand five hundred in 2013 down from more than twenty thousand cases at the start of the project. The national goal of malaria elimination by 2020, in the light of current developments, is attainable. Fine tuning the program strategies and activities can be f urther enhanced with the data gathered in this study. Review of Literature Insecticide treated nets (ITN) are an effective tool for preventing the transmission of malaria[Lengeler C. Insecticide-treated bed nets and curtains for preventing malaria. Cochrane Database of Systematic Reviews 2004, Issue 2.]. Efforts promoting the use of LLIN are geared towards the broader objective of universal coverage, defined at the household level as the use of insecticide-treated nets by all household members regardless of age or gender, and with a coverage at the community level reaching at least 80% of those at risk [WHO, Insecticide Treated Mosquito Nets: A WHO Position Statement]. In a bednet utilization survey conducted in 2012 by Dela Cruz et. al, ninety-eight percent (98%) of households in the areas surveyed owned a mosquito net. On the average, there were 2.72 or roughly 3 nets present in every household. The survey revealed further that eighty-four percent (84%) of total sample population have slept inside a mosquito net the previous night. Also, approximately ninety-seven per cent (97%) of children 5 years old and below in the sample population slept under a mosquito net the previous night and approximately eighty-eight (88%) of pregnant women in the same sample population did likewise. This current survey seeks to determine the current rates of use and roughly validate such practice as revealed in the previous surveys. Objectives of the Survey: The survey aims to: 1. To determine the coverage and usage of LLINs by general populations, by pregnant women, by children less than five years of age in the target provinces, municipalities, and barangays identified. 2. To assess the interviewees’ knowledge, attitudes, and practices with respect to malaria. 3. Generate data for program monitoring and evaluation. The results of this survey will likewise be used as baseline for the 13 provinces to which change will be evaluated against in 2016. Key Indicators to measure: Proportion of population who slept under the mosquito net the previous night Proportion of children under 5 who slept under the mosquito net the previous night Proportion of pregnant women who slept under the mosquito net the previous night Percentage of population at risk covered by LLINs Proportion of population owning a mosquito net (ITN/ LLIN) Proportion of population who know the cause of, symptoms of, treatment for, and preventive measures for malaria stratified by people owning and not owning nets Study area and study population: The BUS-KAP will be conducted in a total of 41 malaria endemic provinces in the country. The 41 provinces will be divided into 2 groups of provinces with each cluster having a sample obtained from them. One group will be composed of 13 provinces while the other group will be of 28 provinces. The 13 provinces in the first group represent the provinces that will be covered in 2015 to 2017 under the GFATM New Funding Model (NFM) Grant. These are the provinces with a three-year average API of more than 1 per 1,000 pop. In these provinces, there are approximately 3,352,465 people living in the stable, unstable and, sporadic transmission barangays (population-at-risk). The second group of provinces of 28 is those with a three-year average API falling below 1 per 1,000 pop. These provinces will no longer be fully covered in the NFM Grant in 2015 to 2017. These provinces have a total at-risk population of 3,094,007. Study Methodology: The study will be a cross-sectional survey. The 41 provinces will be divided into 2 groups – one group with 13 provinces and the other group with 28 provinces. The group of 13 provinces are those with average 3-year API of 1 per 1,000 pop and above; the group of 28 provinces are those with a 3-year average API of less than 1 per 1,000 pop. The group of 13 provinces represents the provinces that will be covered under the NFM Grant in 2015-2017. From each group, only the barangays with stable, unstable and sporadic transmission will be included as source of the household samples. By definition, the people residing in these barangays within the provinces are the population-at-risk. Sample size will be computed using the estimated population-at-risk and household number in the identified stable, unstable, and sporadic transmission barangays. Both groups of provinces will have a sample of their own. The household will be the unit to be sampled or surveyed. The household number in the barangays will be obtained by dividing the population by 5, which is the estimated household size. The barangays in each group of provinces will be clustered according to stratification – a cluster of stable transmission barangays, a cluster for unstable, and another cluster for sporadic transmission. The household samples will be taken from the clusters of the stable, unstable, and sporadic transmission barangays in the provinces. A sample will be taken from each stratification area – a number from all stable transmission barangays, another number will be taken from the unstable, and another from the sporadic areas. The number of barangays per stratification area that are to be included in the survey is 30 (the minimum number statistically requested). and average number of households to be surveyed in each barangay is equal to total number of households per stratification area divided by 30. The barangays to be surveyed will be randomly selected. Likewise, the households to be interviewed in each barangay will be randomly selected. Sample Size: The sample size figures were produced using StatCalc of Epi Info 7. The factors considered in determining sample size are the following: (1) A desired confidence level of 95% (2) An assumed prevalence of 50% in the target population. (3) The Design Effect (DEFF), which expresses the increase of the sample size needed over that of a simple random sample as a multiplier. A DEFF of 2 will be used. (4) The desired precision, typically equal to one half of the confidence interval width. We will assume that an overall precision of 5% is desired. (5) A 10% non-response rate is also assumed Data Gathering: A team will be trained to conduct the interviews among the selected samples. The survey questionnaire form, attached as Annex B, will be used in the interviews. Two sets of questionnaires will be utilized. The first set is the household-level questionnaire that will be answered by the head of the household or, in the event that the head of the household is not present, the person next in authority who residing in the household will be interviewed. The questions to be answered pertains to household composition, characteristics of the house, household health seeking behavior, net ownership, net use, and others (refer to Annex A Questionnaire). The second set is the individual-level questionnaire that will be answered by all eligible adults ranging from 15 years old and above who are residing in the household and are present at the time of visit. The questions in this set pertain to knowledge and attitudes of the respondents. The trained team of interviewers will fill-up the printed questionnaires with the answers provided by the respondents Inclusion Criteria: For the household level questionnaire, the household head, father or mother, will be the eligible respondent. In the absence of either, any resident of the household above 15 years old may answer. For the individual level questionnaire, all individuals residing in the household at the time of survey who are above 15 years old are eligible respondents. Data Management and Analysis: The data from the completed questionnaires will be entered into excel software or other capable data management software. Means and proportions will be computed/ generated to establish the values for the identified indicators: Proportion of population who slept under the mosquito net the previous night Proportion of children under 5 who slept under the mosquito net the previous night Proportion of pregnant women who slept under the mosquito net the previous night Percentage of population at risk covered by LLINs Proportion of population who know the cause of, symptoms of, treatment for, and preventive measures for malaria Ethical Considerations: Confidentiality will be observed and maintained at all times possible. Data gatherers and encoders will be made familiar with the questions and its implications and will be trained on delivering the questions and handling responses gathered. Wherever possible, responses are coded and known only to the research team. All these measures are to minimize the risk of breech in confidentiality that may produce negative consequences to the respondent/s reputation or status. Further, informed consent will be utilized for the respondents of the survey. The respondents will be briefed on the purpose and process of the survey and only if they fully understand and agree to what was explained will they be considered as participant. This will be documented with a signed informed consent form. The data gatherers will likewise be trained in obtaining the informed consent form. Where appropriate, the questionnaires and informed consent form will be translated to the local dialect or dominant dialect understood in the community. The translated versions will be tested for understandability or validity. Review and clearance from a recognized Ethical Review Board will be sought. In addition, review and affirmation from the National Commission on Indigenous Peoples and other governing groups of the affected populations will be obtained as needed. Time Frame: The study will take about 4 months to complete and will be contracted out to a third party entity who will closely work with the malaria Technical Working Group spearheaded by the National Program Coordinator of the Department of Health. The Effects of UFO's on People Almost every civilization in history that has kept a written history has recorded the sightings of strange objects and lights in the skies. These objects have been described as glowing wheels, colored balls of light, and disk shaped objects. Today unexplained aerial phenomena are generally referred to as unidentified flying objects (UFO's) or flying saucers. The effects of extra terrestrial sightings can have an adverse effect on people if influenced the wrong way. No solid proof has shown that UFO's are real, but many sightings cannot be proven otherwise. The media, stories, or one's own experience may often influence what one believes. The government once stated that they had captured extra terrestrials. (60 minutes, CBS) On the contrary, most scientists now agree that almost all sightings and stories of extra terrestrials are not authentic. The United States government has records of thousands of UFO sightings since 1948, including photos of alleged UFOs and interviews with people who claim to have seen them. Since UFOs were considered a potential security risk, the report on these sightings was originally classified as secret. (Craig, 917) When the report was later declassified it showed that 90 percent of all UFO sightings could be easily explained. Most of the sightings turned out to be celestial objects, such as stars or bright planets like Venus, or atmospheric events such as auroras or meteors falling through the atmosphere. Many other sightings turned out to be objects such as weather balloons, satellites, aircraft lights, or formations of birds. Often these sightings were accompanied by unusual weather conditions. Only 5.6 percent of these cases were not explained. Testimonies by people are often very inaccurate and dramatized. People have the tendency to explain everything they see, which is not usually completely accurate. The unaided human eye can be tricked into hallucination and has an inaccurate depth perception. Reflections from windows and eyeglasses can provide an optical illusion of a UFO. Radar is much more reliable to identify objects, but it cannot detect many characteristics that separate natural phenomena and physical objects. There are many ways that people are stereotyped in the world today such as being African American (Black), Sexism, and being a homosexual. Stereotypes have gone to the extreme in today’s society and issues must be addressed. Black people are always stereotypes as being from the projects, or being poor and involved with drugs. Not all black people are like that. Just as with every other race that has its downfall of people in poverty so does black people. blacks try to counter the hate and hostility that they encounter with warmth and kindness toward one another such as with Bill Cosby and the Cosby show. Cosby had hoped that the eight season run of a â€Å"nationally treasured† show about an â€Å"educated, professional, intact Black family† would end stereotypical portrayals of African Americans. But still Blacks are portrayed as inferior, unimportant, and void of credibility. But stereotyping doesn’t end there as sexism. Women are always stereotyped. People say such things as "girls are not boys" to "girls are inferior to boys" and thence to "girls should be discriminated against or confined". Women also encounter sexism in the workplace. It is certainly true that there are very few women managers, however this is just a symptom of the general situation of women as a whole, not a cause. The installation of women at the top of a profession won't change the basic ground rules by which society is run. Those women at the top may suffer sexism from their colleagues. They may be ostracized from the old boys network and may find it more difficult to succeed. Women will remain as second class citizens as long as they are relegated to an inferior position in the work force. They are now in that position because to the bosses they are an unstable workforce, likely to want pregnancy leave, likely to come in late if a child is sick, likely to require a crà ¨c he or want to work part time. It is because men in society are seen as the breadwinner that they have more secure, more dependable jobs. When you come down to basics equal education and job opportunities and equal pay amount to little without free 24 hour nurseries and free contraception and abortion on demand. While a small minority of women can buy control of their own fertility, for the majority, family and child care is still as it has always been the largest problem faced by women workers.

Architecture of the ancient Roman Empire is considered one of the most impressive of all time. The city of Rome once was home to more than one million residents in the early centuries. The Romans had a fine selection of building monuments in the city of Rome including the forums for civic services, temples of worship, and amphitheaters for recreation and play. The Romans made great use and pioneered great architecture mechanisms including arches, columns, and even mechanical elements in pulleys and early elevators. However, when one tends to think of great buildings, one building stands out in Rome. This building is the Flavian Amphitheatre, or better Known as the Coliseum. The Coliseum is the greatest standing building of Rome, and one of the most recognized worldwide architectural achievements to this day. The amphitheater is a type of architecture that was without Greek precedents. This makes sense since its primary purpose was to hold gladitiator fights and brutal shows which were banned in Athens at the time. Such events held in Roman amphitheaters were horseracing, gymnastics, mock cavalry battles, footraces, prizefighting, wrestling, fights between animals, between men, animals and men, and even naumachiae, or mock sea battles. The great building although fitting and plain in design to its surroundings of Rome still stood out due to its sheer monstrosity and oval shape. Although the site viewed today is still a marvel, back in the days of its prime it was a spectacular site that would be difficult to apprehend with only words. The emperor given credit for the idea of the coliseum was Vespasian. Building commenced around 72 AD. But Vespian would not live to see his greatest accomplishment finished. Titus, Vespasian's son, completed his fathers dream around 80 AD. The dedication of the Coliseum was a lavish gladiator show that lasted for exactly one hundred days in which over nine thousand animals were killed. A typical day at the Coliseum show usually started with a bloodless comic relief battle, often times with dwarfs, women, or cripples battling with wooden objects. The gladiator fights were the most popular and prominent fights. These featured two highly trained men battling for courage, strength, and dignity. They would often rather take a blow and stand strong than whimper and run in defense. It is written that famous women would even leave their husbands for famous gladiators, which were known to be very scarred and ugly by Roman standards.

Civil Liberties, Personal Freedoms, and US Law Enforcement Essay

Americans would like to believe that they have freedoms that are available to people in no other country on earth. In a very real way, this statement is true. We are free to criticize the people who govern us in a manner unavailable in most countries, without expecting retribution. Public access is available to many, if not most government buildings. The American process of governing is relatively transparent. All of these things have created a great expectation of freedom, or at least liberty, in this country. And yet, the nature of liberty and freedom, as well as how these thing interact with law enforcement, are not well understood by many Americans. Freedom vs. Liberty Although the concepts of liberty and freedom are related, they are not the same. The United States of America, as noted above, allows many kinds of freedom. There is freedom â€Å"from† things, such as absolute government control of lives and freedom â€Å"to† things, as in freedom to choose a school or a career, rather than having such a thing assigned. Freedom consists of the ability to move about at will, personal liberty, and civil liberty. In philosophical terms, freedom is the â€Å"power to exercise choice and make decisions without constraint from within or without; autonomy, self-determination (â€Å"freedom,† n. d. ). Although liberty is used as part of the definition of freedom, they are not identical concepts. Liberty, however, is freedom from such things as outside control in an individual’s personal choices, from captivity, and â€Å"freedom from arbitrary or despotic government or control† (â€Å"liberty, n. d. †). It can be seen, then, that of the freedoms that Americans expect to have are composed of both freedom–the freedom to†Ã¢â‚¬â€œand liberty–the â€Å"freedom from. Without civil liberties, Americans would find themselves facing an increased number of forcible actions from their own government, as well as potential attempts to infringe upon those liberties from outside. Americans and Civil Liberties Americans have always taken their civil liberties seriously. State seals and mottos refer to liberty. The New Jersey state motto is â€Å"Liberty and Prosperity,† while text on the Massachusetts Great Seal once read, â€Å"By the sword we seek peace, but peace only under liberty. † Patriotic songs refer to the nation as being the â€Å"sweet land of Liberty. From the beginning, then it can be assumed that Americans have expected â€Å"freedom from,† liberty, particularly freedom from tyranny. In today’s world, the concept of tyranny can take many forms. Racial profiling is often considered to be one form of tyranny, although it is not always simple to assess when it occurs (Barkan, 2005). Research from the Institute for Public Policy and Social Research [IPPSR] indicates that Americans largely favor the protection of civil liberties, even when responding after a crisis. However, the degree to which people are willing to accept changes to policies that affect civil liberty will vary based on race and ethnicity, political ideology, the area in which they live, and more (ISPPR, 2002). One significant influence on whether these changes should occur is that of trust in law enforcement (ISPPR, 2002). According to xxxx (2002): citizens with low trust in local law enforcement give 6. 7 (out of 8) pro-civil liberties responses, while citizens with moderate trust give 5. 4 pro-civil liberties responses, and citizens with high trust give only 4. 7 pro-civil liberties responses. p. 4) These findings are significant to this paper in that the lack of trust in local law enforcement seems as if it is often both the cause and the result of the effects of policing on the community. If people have contact with policing agencies that allows them freedom to do something, then they feel comfortable with the amount of civil liberty that they have and may consider adjustments to that liberty. If individuals in a society feel as if they already have little freedom, then they will necessarily feel as if the amount policing that is permitted should not vary, at least not by increasing it. New Policing† The methods for policing seem to be changing worldwide, shifting from public policing to private policing. These changes seem to be occurring primarily in democratic societies, which have looser control over individuals’ lives. According to Ashcroft (2001), democratic governments can accommodate diverse centers of power, whereas authoritarian regimes cannot. Democratization facilitates restructuring by providing political space into which it can grow. As political pluralism increases, so too do the auspices that want to share responsibility for policing. (p. 28) Why, then, are democratic nations more difficult to police than others might be? The answer appears above: authoritarian regimes maintain control of their citizens by coercion and force, which does not permit decentralized policing of their nations. They do not have the freedom from, not in many capacities, and for this reason their citizens are not at liberty to act or to decide what changes that the government that can affect their lives. ecause they do not have the freedoms â€Å"from† that Americans have, the government can decide what police actions must be taken, when they will be taken, and what form these actions will take. The reason that Americans are so shocked by such events as the Kent State actions, the actions taken at Ruby Ridge or those taken against the Branch Davidians, is that they appear to be so totalitarian. Americans expect better of their le aders and respond with words that indicate they feel betrayed and, sometimes, those betrayed feelings result in votes being withheld. Democratic nations are so difficult to police because their policing mechanisms are so diverse and are so dispersed. In addition, actions taken against the citizens require a reason and a precedent, unlike policing actions in authoritarian or totalitarian states, which need no apparent precedent to act. Conclusion Democratic nations require balance between the citizens, their government, and their policing agencies. Citizens must feel that they have value and that they are safe from unprecedented police actions, even from their own government. Such nations are difficult to police because only authoritarian nations is it possible to have access to the â€Å"easy† method of policing that involves force. While it is not impossible even for a democratic nation to have policing actions that involve force, they incite comment because they are unusual and frequently cause outrage. Policing a democratic nation is difficult because democracy is hard. To accept policing that negatively affects either freedom or liberty–or both–results in a nation that has fewer civil liberties, as well.

Agricultural Revolution of the 10th Century

This paper will look at the Agrarian revolution that occurred in 10th century and the resulting effects on the political, social, cultural and economic conditions in Europe.Agricultural revolution in Europe was a period where there was witness of sustained changes in the way people carried out their farming activities. It had to do with increased output of products and general changes in the methods of carrying out agricultural activities on farmland.  10th century Europe fell under the â€Å"Middle ages, an era when agricultural methods of production were mostly peasantry in nature. Most of the populations during the 10th century Europe were serfs who were employed by other people to work on their farmlands† (Chris, par. 2).There was no clear sub-division of land and the main tool farming was the ox drawn plow and the system of farming was the two field system. This was a system of farming where one field was cultivated while leaving the other one fallow so as to reclaim th e soil’s nutrients. Population growth during these ages was very low. This can be attributed to the poor methods of farming which led to low yields hence food insecurity.Agricultural revolution came about with the systematic changes in the methods of production. One major indicator of the revolution was through the consolidation of common land that led to introduction of individual ownership of those properties.Thus rights of ownership were created through the abolition of the open field system of cultivation. The plow as the main tool of cultivation underwent redesigning, and also, the two-field crop rotation was changed to a three- field, and hence these served as contributors to the increased food production that occurred.The Agricultural revolution that occurred at this time brought great impacts to the socio-economic conditions of Europe as a whole. One impact was increase in population, mostly in the towns. Individual ownership of land led to many people selling off the ir lands.The consequences were that there was increase in the number of people without land, but who were working as laborers for the big landowners. Others migrated to towns and thus contributed to rapid population growth.Furthermore adoption of better methods of farming led to increased food production and hence surplus production. This surplus production was then traded for other goods that were lacking. This served as the beginning of exchange of goods and services and also development of trading systems.The agricultural revolution also served as the main cause of change from feudalism to capitalism. Introduction of wage labour whereby landlords entered agreements with workers served as the springboard on which market economy fundamentals were established and power over the land which was appropriated by those who controlled it.Developments that resulted from the agricultural revolutions in terms of new towns, migrations and changes in land ownership necessitated changes in the political and cultural system of the people.There were changes from â€Å"feudalism as the most dominant form of political organization to formation of the nation-state† (Adriaan, par. 4).
